  How can I pay my premium?
If you are paying monthly all you need is a voided check. Your account will be billed on the 20th of the month prior to effective date. If you are paying quarterly you must collect a check for 3 months of premium. If you are paying semi-annual or annual you must collect a check for the total # of months up to the date of the rate change(Health-Jan 1, Med. Supp.-April 1), and attach a letter stating to be billed annually from this point forward.

  Can I change a requested effective date?
If you requested an effective date on the application then you will have to do a new application to change the effective date. You may write in on the application no health history has changed since first application. If you did not request an effective date then we will just need a letter in writing stating you want to change the effective date.

  What is the difference between the BluePreferred Plan and the Value Plan?
The BluePreferred Plan has maternity benefits automatically built into the policy, where the Value Plan has no maternity benefits at all. The BluePreferred Plan also has a $300 accident benefit, Inpatient, and Outpatient treatment for mental illness, drug abuse and alcohol. The Value Plan has no accident benefit and no Inpatient treatment, but it does include Outpatient treatment.

  What does standard and preferred mean?
This is simply a tobacco or nontobacco question. If you have not used tobacco in the last 12 months then you qualify as preferred rates. If you have used tobacco in the last 12 months then you qualify for the standard rates.

  What forms do I need to submit with my application?
For an Under 65 application you need the application # 3087 Rev. 01/22/2004. If an employer is paying the premium you will also need form # 3117. For an over 65 application you need the application # 31-041 Rev. 09/22/03. If you are replacing another Medicare Supplement you will need form # 3205.

  What age do I use to figure the applicant's rate?
For an under 65 application you will use the age the applicant is on 1/1/2005. For an over 65 application you will use the age the applicant is on 1/1/2004, unless they are just turning 65 in which case you will use the 65 rate.

Blue Cross does provide 24 hour coverage to those individuals who are not required to purchase workmen's compensation insurance (business owners, farmers, etc.).
