Eligibility and Tax Documentation

Tax Documentation Requirements  

 Small Group New Business (Fully Insured) 

 

Blue Cross and Blue Shield of Nebraska Logo 

-          Need minimum of TWO W2 non-owner/partner/spouse employees enrolling 

-          $0 commission on two person groups 

-          Tax documents: Quarterly wage/tax report, W3 + W2s. Schedule C, Schedule F or K1s for eligible owners in coordination with the other tax documentation. 

-          Participation: 100% for 2-3 eligible employees, 100% minus one life for 4-9 eligible employees, 75% for 10-50 eligible employees after valid waivers. 

 

  

-          Need a minimum of ONE W2 non-owner/partner/spouse employee enrolling 

-          Tax documents: Quarterly wage/tax report or two weeks of 3rd party payroll. Schedule C, Schedule F or K1s for eligible owners in coordination with the other tax documentation. 

-          Participation: 70% after valid waivers. 

-          1099s eligible if there is at least one W2 employee enrolled

-          Need a minimum of TWO employees enrolling, but 3 eligible (husband and wife allowed if 3 eligible) 

-          Tax documents: Quarterly wage/tax report, K1s, 1099s, payroll reports 

-          Participation: 75% after valid waivers (and 50% floor participation) 

-          1099s eligible 

                             

FUNDING ADVANTAGE 

-          Need a minimum of TWO employees enrolling 

-          1099s NOT ELIGIBLE 

-          Cities, villages, counties, churches, religious organizations NOT ELIGIBLE 

  

ALLSAVERS 

-          Need a minimum of FIVE employees enrolling 

-          Tax documents: Quarterly wage/tax report or two weeks of 3rd party payroll. Schedule C, Schedule F or K1s for eligible owners in coordination with the other tax documentation.
